What to Expect During Your First Visit
Before your appointment you will receive a New Patient Packet by mail, or you can download the forms online.
If you cannot download and print it in advance, you may complete your paperwork once you arrive at our office.
Please arrive 20 minutes early if you need to complete paperwork in the office.
During your visit we will:
- Review your medical history and medications
- Ask about allergies and current health concerns
- Request a urine sample for testing
- Ask you to sign consent to our Notice of Privacy Practices (PDF)
